I want to have my patients sign consent forms that are created in Microsoft Word. Within the consent are also some sections where patients have to check off their choices. I also have them initial each page. At the end, up to three people sign the consent, not always at the very same moment.
I am looking for software that will interface with Word and store the initials, check marks and signatures with the document or document template.
Lastly, if there is a significant recommendation regarding a tablet PC, I am open to suggestions. Thanks! |

Hi,
Word supports check boxes. It's easier if you have the Forms Toolbar enabled - just click on the Check Box icon and it will be inserted in the Word document where the cursor is.
As for the ink, you can insert places where the user can draw with the stylus using Insert Ink Comment. Then you can resize it to whatever size you want. _________________ Chris De Herrera

our company is thinking about going paperless and what you will find is the leagilty issues based upon electronic sigs. What we found in our reaserch is in order for it to be a legal binding document to stand up in court in needs a dig cert. The program we chose to use is adobe acrobat. With a word doc you could simply print the file as a pdf and have them sign that print each one would then be a legally binding documnet. As far as i know adobe set the standard for this. Not sure if anyone else does it cause it cost us a pretty penny to buy adobe |

You don't need Adobe to convert a document to a PDF.
Perhaps this will help? This program setsup like a printer driver and when you use it, it creates a PDF.

yes you can use print to pdf but the problem you are all forgetting is medical practices have to stay hippa compliant and the onlt way to do so, is with a digital sig |
